Is Antimicrobial Susceptibility Testing Necessary in the First Helicobacter pylori Eradication?

Abstract

|
Background/Aims: Resistance to antibiotics is one of important factors in the eradication of Helicobacter pylori. However, there are debates about the necessity of the pre-treatment antimicrobial susceptibility testing. Thus, our aim of this study was to evaluate the necessity of antimicrobial susceptibility testing before the first eradication.
Methods: We retrospectively reviewed 223 consecutive patients who were performed Helicobacter pylori culture by endoscopic biopsy and received PPI-based triple therapy in Asan Medical Center from October 2004 to August 2009.
Results: In 145 patients, culture was performed before first eradication and in 88 patients, culture was performed after failure of first eradication. Among 145 patients, all drug susceptible cases were shown in 55 patients and susceptibility test was not performed in 74 patients. Success rate was 89% in all drug susceptible group and 80% in no testing group (p=0.154). Resistance rates to clarithromycin and metronidazole were 7% and 14% in pre-treatment group, but in treatment failure group, rates were noted in 73% and 53%.
Conclusions: These results indicate that antimicrobial susceptibility testing before the first Helicobacter pylori eradication is not effective method. However, resistance rate was higher in treatment failure group, so we can consider about performing susceptibility testing after first treatment failure.
